After booking, you'll receive intake forms via the SimplePractice client portal. Complete them at least 24 hours before your appointment so Lori can review them in advance.
Well-I-AM accepts Aetna, Medicaid, Medicare, Cigna Behavioral Health via Evernorth, and Imperial Health Plan. Self-pay rates are also available — $120 for the initial appointment, $80 for monthly follow-ups. We accept all major credit cards and HSA/FSA cards.
